The patient needs to recognize what they have to do, when they should do it, and how to resolve a doubt. A useful summary turns the agreed upon into clear steps without adding new instructions.

The work begins in the conversation. The document reinforces what has been explained; it should not introduce a major change for the first time or force the patient to interpret abbreviations in the file.

Write for someone who will read outside the office

Imagine that the summary is reviewed hours later, without you present. Replace expressions like «as discussed» with the agreed-upon action. If the plan includes a study, identify the study and the next step indicated. If it includes a review, write how it will be organized according to the actual procedure of the consultation.

Do not fill in the spaces with general recommendations. If a given point is not defined, resolve it before submitting the document. A well-presented text can give the appearance of a decision when it still remains to be decided.

Order by actions, not by the format of the professional note

You can organize the content into three parts: what was explained; what the patient should do; and what will happen next. Add other sections only when the meeting requires them. A follow-up visit does not necessarily require the same document as a first assessment.

For example, «bring the previous reports for the next review» is more direct than «a request for complementary preclinical background information.» The change is linguistic: it preserves the action and avoids requiring the reader to decipher terminology. It does not replace precision with a simplification that changes the meaning.

Separate existing treatments from new changes

When there are indications of medication, avoid mixing what the patient was already using with what was decided on during this visit. The summary should exactly reflect the revised plan, including the details you have indicated. Do not deduct doses, duration, or suspensions from a list of previous prescriptions.

If the recipe and the summary contain the same instructions, they must match. This verification is especially useful when both are prepared from the same context using digital tools. A discrepancy between documents raises a question that could have been resolved before they were submitted.

Check understanding without examining the patient

The technique of teach-back described by AHRQ It consists of asking the person to explain in their own words what they need to know or do. You can start by asking them: «To check that I explained it well, how will you organize these steps when you get home?».

The question tests his or her explanation, not the patient's ability. If the answer reveals confusion, reformulate the question and also review the text that the patient will deliver. Asking only «Did you understand?» provides less information about where the confusion came from.

Take care of language, format, and channel

Confirm which language you find most comfortable to read in. Use short sentences and separate different actions. If another person will help with the plan, agree on how they will participate and what information should be shared. Don't assume that a family member knows the context just because they are present at the end.

Choose a channel that is authorized by your query and appropriate for the patient. Check that the file opens, that the text is readable on the phone, and that there is a clear way to ask questions. The design should help you find the next action without having to scroll through a long block of text.

Where AI can help

An assistant can prepare a first version in a more accessible language based on the documented context. The benefit is to avoid writing from scratch and maintain consistency with the query. The final review should focus on ensuring that the summary retains the decisions and does not incorporate additional instructions.

You can check how they work summaries for patients in Itaca and its configuration of Preferred language of the patient. The goal is simple: that the person leaves with an explanation that they can use, in addition to a document that they can keep.

Organize the instructions into easy-to-follow actions and review their explanation without adding decisions that were not part of the consultation.

Organize the instructions into easy-to-follow actions and review their explanation without adding decisions that were not part of the consultation.
"_jetpack_memberships_contains_paywalled_content":false
_jetpack_memberships_contains_paid_content":false
[15
28,9
post-5151","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-guides","tag-medical-documents","tag-patient-visits
https://i0.wp.com/itaca.ai/wp-content/uploads/2026/10/itaca-editorial-instrucciones-pacientes.webp?fit=1536%2C1024&ssl=1
["GET
