Artificial intelligence in medicine in Nicaragua
In Nicaragua, clinical artificial intelligence must be integrated into an organized network through the Ministry of Health and the Local Integral Health Care Systems (SILAIS). Regulation 004 establishes how to handle the clinical record in public and private providers, while Law 787 regulates the processing of personal data.

Medical record, privacy, and AI in Nicaragua
Standardized clinical record
MINSA Regulation 004 seeks to standardize the structure and recording of medical records in public and private facilities. It defines it as a care, educational, research, and medico-legal instrument. MINSA: Regulation 004 for the medical record
Problem-oriented documentation
The national manual includes the problem-oriented medical record, and the General Hospital Regulations indicate the use of the SOAP methodology in hospital management. This makes the structure, authorship, signature, and professional review especially important. MINSA: General Hospital Regulations ↗
Protection of personal data
Law 787 protects privacy and informational self-determination against the automated or non-automated processing of personal data in public and private files. Its specific application must be reviewed in conjunction with health regulations and the establishment's policies. National Assembly: Law 787
Clinical documentation challenges in Nicaragua
The existence of a national standard does not mean that all establishments use the same digital infrastructure. Adoption must consider the network, the level of care, and the actual flow of the file.
- Maintain consistent, organized, legible, and intelligible records across services and levels of care.
- Prevent an external tool from creating parallel documentation that does not reach the institutional file.
- Identify who prepares, reviews, signs, corrects, and keeps each note.
- Protecting data when cloud services, vendors, or information transfers are involved.
Reference hospitals in Nicaragua
The MINSA network identifies hospitals with national referral services. These institutions help understand the complexity and specialization of the country's clinical practice; the selection is not a ranking nor does it imply affiliation with Itaca.
Antonio Lenín Fonseca Teaching Hospital
Managua. Teaching hospital with national reference services and specialized care.
Manuel de Jesús Rivera Children's Hospital “La Mascota”
Managua. Pediatric hospital with national referral services.
Bertha Calderón Roque Hospital
Managua. National referral hospital for women's healthcare.
SILAIS connect a network; they do not make their records identical
The SILAIS territorially organize establishments and levels of care, but the document flow still depends on the service, technological capacity, and institution. A national hospital, a social security clinic, and a health post do not necessarily produce or exchange information in the same way. Normative 004 offers a common documentary basis; it does not demonstrate by itself that the entire network operates with an interoperable electronic health record.
To introduce AI, it is advisable to choose a specific note or document and check its correspondence with the required structure: identification, clinical problem, evolution, authorship, and review. If audio, uploaded documents, or cloud services are involved, the organization must also map who accesses, what is retained, and how it is deleted. Adoption adds value when it reduces transcription and pending tasks without separating the draft from the institutional record.
What to evaluate before adopting clinical AI
A Nicaraguan organization should start with the document and the workflow it wants to improve, not with the AI model. The test must show how professional accountability is preserved and how the reviewed content reaches the official record.
Documentary quality
Verify that the output complies with the structure, terminology, and fields required by Regulation 004.
Professional control
Keep review, correction, signature, and seal when applicable to the document.
Privacy and traceability
Define purpose, access, retention, transfer, and change log.
Operational integration
Ensure that the result fits into the institutional system and the workflow between services.

ITACA IN NICARAGUA
Drafts that fit the establishment's workflow
Itaca can help turn conversations, documents, and quick notes into structured clinical drafts. The professional reviews the content, and the institution retains responsibility for its medical record, policies, and systems.

Frequently asked questions about medical AI
Does Regulation 004 mean that all of Nicaragua uses the same electronic system?
No. It standardizes aspects of file management, but the infrastructure and specific workflow must be verified in each establishment.
What is the relationship between SILAIS and the clinical record?
The SILAIS organize the network territorially. The file and its procedures continue to depend on the level, service, and institution.
Can inquiry audio be used without reviewing the data flow?
It should not be done. Purpose, access, retention, provider, and review procedure must be defined before using real data.
How can Itaca help?
Itaca can convert the consultation and other documents into structured drafts. The professional reviews them and the institution determines their inclusion in the file.
