Environmental scribes prepare documentation based on clinical conversations. Their proposal is easy to understand: that the professional devotes less attention to writing while attending to patients. What requires a more careful reading is the evidence on how much they help, in what situations, and with what review work.
The useful question is not whether “AI saves time” in general terms. The question is what tool was studied, how it was incorporated, what result was measured, and how much that environment resembles the query. Two similar headlines can describe very different evaluations.
An example of evidence in ambulatory practice
A study published in JAMA Network Open in 2025 He followed 46 professionals from 17 specialties in an outpatient academic setting in Philadelphia. He evaluated DAX Copilot, not Itaca, by means of a pre-and-post comparison without a control group. He found improvements in documentation metrics and professional perceptions.
The result is relevant because it observes healthcare work, but the design does not allow us to attribute all the change to the product alone. It also does not establish what will happen with another tool, another language, or a query with a different infrastructure. Its main usefulness for the reader is to show what is worth measuring, in addition to providing a result in that context.
Time, perceived workload and quality are not the same variable
Spending less minutes on the note, finishing less work outside of working hours and feeling less burdened are related benefits, but different. One evaluation may show improvement in one area without demonstrating it in others. Ask how each data was obtained: system log, stopwatch, questionnaire or retrospective estimation.
Documental quality requires its own evaluation. A note can be produced faster and require significant corrections. It can also be longer without providing useful information. Experience of use does not replace content verification, just as a correct note does not demonstrate a reduction in workload.
Integration is part of the result
Observe where the studied flow ends. Does the note reach the usual file or does the professional have to transfer it? Was a template configured for the service used? Was training and support provided? These conditions can change the experience, even though the transcription function may seem similar.
For an independent consultation, a well-integrated system in a hospital is not an identical reference. That does not invalidate the study: it limits its application. It is advisable to describe the complete circuit, from initiating the capture to leaving the note at the place where it will be used.
What to look for before transferring a result to Latin America
Check the language, specialty, length of consultations, connectivity, and how to save the documentation. Don't assume that sharing the language solves differences in vocabulary, accents, forms, or local practices. These are evaluation questions, not tests of whether a tool will fail.
Also include meetings where the system could not be used or was not wanted to be used. Analyzing only the completed sessions can leave out part of the daily work. Information about actual usage helps interpret an average that, on its own, seems more conclusive than it is.
What a documentation study does not prove
A reduction in writing time does not in itself demonstrate better clinical outcomes, fewer treatment errors, or greater profitability. To support each claim, appropriate measures are required. Furthermore, it is not appropriate to transfer the results of one product to the entire set of available assistants.
When reading ads or reports, look for simple correspondence: each conclusion should have a measure that supports it. If satisfaction is measured, the result is satisfaction; if time is measured, specify what time. This precision allows us to recognize progress without turning it into universal promises.
How to bring evidence to a practical decision
Try a limited flow and record active time, corrections, and pending tasks. Compare similar encounters and save the less favorable results. The clinical notes with AI They should be evaluated as part of that complete work, not just for the speed of generation.
In our Reports on AI We distinguish the analysis of public sources from the observations within the product. This separation matters: knowing that a function is used helps understand adoption, but it does not in itself demonstrate its impact on the consultation.
